INFORMATION FROM THE EPO
Notice dated 27 February 2006 concerning updated EPO Forms 1010, 1037 and 10381
The EPO has updated a number of its forms, and recommends that applicants now use these new (02.06) versions of
- EPO Form 1010 (Payment of fees and costs)
- EPO Form 1037 (Acknowledgement of receipt for subsequently filed items)
- EPO Form 1038 (Letter accompanying subsequently filed items)

The fax number given under the EPO address is only for direct correspondence with Treasury and Accounts. All other faxes should be sent to the EPO's general numbers2.
On the back of the form, Lithuania and Latvia are still on the list of extension states, because they could feature as such in Euro-PCT applications.
Applicants are also reminded that they can now operate their EPO deposit accounts online. This simplifies fee-payment procedures considerably, and is becoming an increasingly popular option. For more details, see http://www.epoline.org.
The new (02.06) version of Form 1010 is reproduced below. Please use it in future, and replace any earlier electronic version you may have stored locally. This will ensure machine-readability at the EPO.
Ordering the forms
All forms are available free of charge from the EPO (preferably in Vienna but also in Munich, The Hague and Berlin) and the central industrial property offices of the contracting states. The latest versions can also be downloaded in PDF format from the EPO's website (www.epo.org → Tool box for applicants).
